- Reach out to their Edvak Account Manager
- Inform them that Surescripts Change & Refill permissions need to be ported
Step 2: Submit Required Information
- The Account Manager will provide the required details and forms needed to initiate a porting request with Surescripts.
- The practice must complete and return the requested information.
Step 3: Edvak Submits Porting Request
- After receiving valid information from the practice, Edvak will submit a formal Change of Vendor request to Surescripts.
Step 4: Processing Timeline
- Surescripts typically takes 5 working days to complete the porting process.
After Porting is Completed
Once the Change of Vendor process is finalized:
